Directions for use: collect faecal specimens

Allow the test, specimen, buffer and/or controls to reach room temperature (15-30°C) prior to testing. Collect faeces in a clean, dry specimen collection container. Best results will be obtained if the assay is performed within 6 hours after collection. Specimen collected may be stored for 3 days at 2-8°C if not tested within 6 hours.

Directions for use: process faecal specimens

Unscrew the cap of the specimen collection tube, then randomly stab the specimen collection applicator into the faecal specimen in at least 3 different sites. Do not scoop the faecal specimen. Screw on and tighten the cap onto the specimen collection tube, then shake the specimen collection tube vigorously to mix the specimen and the extraction buffer. Specimens prepared in the specimen collection tube may be stored for 6 months at -20°C if not tested within 1 hour after preparation.

Directions for use: running the test

Bring the pouch to room temperature before opening it. Remove the test cassette from the foil pouch and use it within one hour. Best results will be obtained if the test is performed immediately after opening the foil pouch. Hold the specimen collection tube upright and open the cap onto the specimen collection tube. Invert the specimen collection tube and transfer 2 full drops of the extracted specimen (approximately 80µL) separately to the specimen well (S) of the test cassette, then start the timer.

Directions for use: reading the results

Read results at 5 minutes. Do not read results after 10 minutes.

Interpretation of results

POSITIVE:* Two or three coloured lines appear. One coloured line should be in the control line region (C) and another one or two apparent coloured line should be in the test line region (T1 and/or T2). *NOTE: The intensity of the colour in the test line region (T1 and T2) will vary depending on the concentration of Human Haemoglobin(T2:FOB) and Transferrin(T1:TF) present in the specimen. Therefore, any shade of colour in the test line region (T1 and T2) should be considered positive. NEGATIVE: One coloured line appears in the control line region (C). No line appears in the test line region (T1 and T2). INVALID: Control line fails to appear. Insufficient specimen volume or incorrect procedural techniques are the most likely reasons for control line failure. Review the procedure and repeat the test with a new test.

Transferrin is more stable than haemoglobin in the digestive tract, making it a useful additional marker for detecting upper gastrointestinal bleeding. Combined FOB and transferrin testing provides broader screening coverage in oncology pathways.

What does the Transferrin & Faecal Occult Blood test detect?

The test detects human haemoglobin (faecal occult blood) and human transferrin in stool samples. Together, these biomarkers help identify gastrointestinal bleeding that may be associated with colorectal cancer, upper GI malignancies or other gastrointestinal conditions.

Why does this test include transferrin as well as faecal occult blood?

Transferrin is more stable than haemoglobin as it passes through the digestive tract. Its inclusion improves the detection of bleeding originating from the upper gastrointestinal tract, providing broader screening coverage than FOB testing alone.

How is this combined test used in screening pathways?

Combined FOB and transferrin testing supports enhanced gastrointestinal screening, particularly in cases where upper GI bleeding may be suspected or where increased sensitivity is required for early assessment.

Is a positive result diagnostic of cancer?

No. A positive result indicates the detection of blood and/or transferrin in the stool but is not diagnostic of cancer. Further clinical investigation is required to determine the underlying cause.

Are dietary restrictions required before using this test?

No dietary restrictions are required. The test is specific to human proteins, minimising interference from food sources or dietary blood.
